Before Bad Medicine is complete, he will be unsure if he is affected by the plague:

"The Maulborn have special healers called nostrums. That's why they were able to develop a cure so quickly.
Feel my head. Do I feel warm to you?"

After Bad Medicine, he has lost hope and wants you to stay away from him:

"They lied to us. There is no cure. Stay back before I give you the plague."
